Timesheet Instructions:
- Use only black or blue ink on your timesheet
- Print client’s first and last name
- Week ending date is ALWAYS Sunday of the week you are currently working
- Print your first and last name
- Write the date for each day of the week that you work
- Check off at least 5 services that you provide for the client each shift
- Arrival and Departure time must be written in military time (see chart)
- Write the total hours worked each shift
- Initial your timesheet after each shift
- Have your client or their representative initial your timesheet after each shift
- Sign and date your timesheet at the end of your week with the client
- Have your client or their representative sign and date your time sheet at the end of the week
- Completed and correct timesheets are due in the office by the WEDNESDAY following the week ending date
To Correct Errors:
- Do Not Use White Out or mark over on your timesheet
- Draw one single line through error
- Make the correction
- Write your initials and date the correction was made next to it
